La douleur court comme un fil rouge, du début à la fin de la vie et de l'œuvre freudienne, aussi bien dans ses aspects physiques que psychiques. Cependant, les disciples de Freud et les analystes contemporains n'ont pas manifesté le même intérêt pour elle, et la médecine moderne la confie de plus en plus à des consultations ou centres spécialisés. Pourtant, comme Freud le pensait, la douleur physique peut être un symptôme psychique au même titre que l'angoisse, la dépression ou les obsessions.
Penser la douleur et essayer de mieux saisir son énigme, c'est faire avancer la compréhension du psychisme humain, en particulier dans son aspect traumatique. Tel est l'objectif de ce livre. Une première partie analyse la douleur dans la vie et dans l'œuvre de Freud. La seconde partie, vouée à la clinique, étudie des catégories nosographiques sous l'angle exclusif de la douleur hystérie, hypocondrie, névrose traumatique dans lesquelles Freud a reconnu une origine psychogénique de la douleur.
Enfin, l'auteur consacre une troisième partie à la théorie et à la pratique de la prise en charge psychothérapique de la douleur quelle que soit son origine (psychique ou somatique). Il introduit la notion de " névrose algique " et étudie les différentes fonctions de la douleur et les remaniements qu'elle impose au Moi pour détailler ensuite les caractéristiques du fonctionnement mental des douloureux chroniques.
De nombreux cas cliniques illustrent ce livre, relevés principalement au sein d'une consultation hospitalière créé par l'auteur à l'hôpital neurologique de Lyon et animée uniquement par des psychanalystes.
